Verification Forms are due by the 5th of each month for the month prior along with the Monthly Payment Cover Sheet.  Late submission of Forms may result in late payment. They can be faxed to 245-1553, mailed to 880 Grandville Ave, Suite B, Grand Rapids, MI, 49503 or e-mailed to alcinterpreters@gmail.com. Large quantities are preferred through the U.S. mail. It is of the utmost importance to have all forms filled out completely and legibly including required signature and approval # for each patient/client in order to issue payment. (Please make extra copies for your own records of the Verification Forms and the Monthly Payment Cover Sheets).
